Open vs Closed Rhinoplasty: Comparing Techniques

Rhinoplasty, often referred to as a procedure to reshape the nose, involves techniques that address concerns related to nasal structure and appearance. Two commonly used approaches are open and closed rhinoplasty, each with its own characteristics and considerations.

In this blog, Ariste Plastic Surgery will provide an overview of these two approaches, including their key differences, general advantages and considerations, and the factors that may influence the choice of technique. It’s important to consult a qualified health professional to determine the most appropriate approach for your unique needs and goals.

Differences in Surgical Technique:

Open Rhinoplasty:

Open rhinoplasty involves an incision across the columella, the strip of tissue separating the nostrils. This technique provides the surgeon with a direct and comprehensive view of the nasal structures. With the skin lifted, the surgeon gains better visibility and precise control over modifying the nasal framework. This approach can be beneficial for complex cases that require significant alterations and detailed adjustments.

Closed Rhinoplasty:

Closed rhinoplasty, on the other hand, employs internal incisions within the nostrils, aiming to avoid any external scarring. This technique relies on instruments and endoscopic assistance to make modifications. It is typically preferred for less complex cases that involve minor adjustments to the nasal structures.

The Potential Advantages

Open Rhinoplasty:

  • Surgical Precision: The direct visualisation offered by open rhinoplasty allows for meticulous modifications.
  • Maybe Suitable for Complex Cases: This technique may be suitable d for extensive nasal reshaping, commonly  for individuals with intricate anatomical issues.
  • Addressing Structural Deformities: Open rhinoplasty gives the surgeon control in addressing  structural concerns such as a deviated septum or cleft lip.

Closed Rhinoplasty:

  • Reducing External Scarring: The absence of external incisions reduces visible scars on the columella.
  • Recovery Considerations: Closed rhinoplasty is often associated with minimal external incisions, which may influence the recovery experience for some individuals. Factors such as the technique used, the extent of the procedure, and individual healing responses can all play a role in the recovery timeline.

The Potential Disadvantages

Open Rhinoplasty:

  • Small Visible External Scar: The incision made across the columella in open rhinoplasty may leave a small visible scar.
  • Recovery Considerations: Open rhinoplasty involves making an incision at the base of the nose, which allows for direct access to the nasal structures. This technique may result in a recovery process that, for some individuals, includes swelling and bruising that can take several weeks to subside.

Closed Rhinoplasty:

  • Limited Surgical Visibility: The lack of direct visualization in closed rhinoplasty might make it more challenging for surgeons to make precise modifications.
  • May Not Be Suitable for Complex Cases: Closed rhinoplasty may not be the ideal choice for individuals with complex nasal issues that require extensive structural changes.

Like in any surgery, both these rhinoplasty surgery techniques involve some potential risks and complications. For personalised guidance and an understanding of what to expect, consulting a qualified health professional is essential. Obtaining a GP referral and undergoing BDD screening are also required before proceeding with any cosmetic procedure.

The selection of open or closed rhinoplasty depends on several factors, including the patient’s individual needs, anatomical considerations, the desired changes, and the surgeon’s expertise.
