Deep Plane Neck Lift

A Deep Plane Neck lift like a Deep Plane Facelift involves operating deep to the muscles on the neck. The neck muscle is the platysma. Dr Jack Zoumaras over 10 years experience on face and neck lifting involves multiple steps (over 10) to delineate and define anatomical neck structures that warrant attention. Deep Neck Lift involves pre and post platysma removal. Digastric muscle trimming and submandibular gland trimming to achieve or gain a 90 degree cervico mental angle. Further to this the cervico manibular angle is addressed and the lateral angle of the mandible is deepened through removal of the tail of the parotid gland.

A Deep plane neck lift is a type of cosmetic surgery that focuses  the appearance of the neck area. It is designed to address issues such as excess skin/rhtyids, excess fat, and loose muscles in the neck and jawline.

During the procedure, the surgeon makes an incision around the ears and jawline, and then lifts and repositions the skin, fat, and muscle tissue beneath the skin to create a cervico-mental-submandibular angles.

This technique is called a “deep plane” lift because it involves working with the deeper layers of tissue, rather than just the superficial layers.

The aim of the deep plane neck lift is to empty the neck contents beneath the mandible which deepens the cervico mental angles independent of any sutures.

Deep Plane Neck Lift Benefits and Risks

The benefits of a deep plane neck lift include removal of excess rhytids and fat, acute cervico mental angle, and reduced signs of ageing in the neck area. It can also improve the face and neck line by addressing the deep submandibular, parotid glands and digastric muscle.

However, like any surgical procedure, a deep plane neck lift does involve some risks and potential complications, such as bleeding, infection, scarring, and nerve damage. It is important to choose a qualified and experienced surgeon and to carefully weigh the risks and benefits before deciding to undergo this type of procedure. 

Dr. Jack Zoumaras offers a neck lift procedure that focuses the neckline for patients with looseness (rhytids) and fullness (excess adipose tissue or fat) in the neck area alone, while maintaining an overall anatomical cheek and jawline. This procedure, known as the Deep Necklift, reworks the deep muscle layers and fibrous tissue of the neck through a single incision hidden in the crease behind the chin.

The ideal candidates are those with good skin elasticity and without significant crepey or loose skin, between the ages of 20-50, non-smokers, in good health, and with a BMI of 31 or under. Patients with a more full or round face may also benefit from a buccal fat pad reduction.

During the consultation, Dr. Jack Zoumaras will examine the patient’s neck to determine if the procedure is suitable and identify specific issues that need to be addressed for optimal results. The Deep Neck Lift procedure is performed under general anesthesia, and patients must prepare ahead of time by filling prescriptions, completing any required testing, and arranging for an adult to stay with them overnight after the surgery. 

It is commonly believed that an undefined neckline is only caused by advanced age; however, this is not entirely accurate. Many patients have never had a defined neckline, even from an early age. Consequently, patients in their 20s and 30s may experience the effects of neck ageing sooner than their peers due to anatomy of the mandible and maxilla.

Such younger patients may be suitable for deep neck work to deepen the cervico mental angle.

Typically, individuals aged 20-50 with minimal skin laxity, no jowling, non-smokers, in good health, and with a BMI of 31 or below make the best candidates for the Deep Necklift.

Patients between ages 50-75 are also suitable but will be assess medically.

For optimal  treatment, this procedure is often combined with buccal fat pad reduction. Some patients have a fuller or rounder face due to larger buccal fat pads.

 An incision is made on each side of the mouth to access the pad from inside, and the pad is excised. Finally, the incision is closed using dissolvable sutures. 

When you come in for a consultation regarding a Deep Necklift, Dr. Jack Zoumaras will begin by asking you to describe the aspects of your neck that you find bothersome. He may also inquire about related areas of your face, including your skin tone, facial hollowing, or cheek position. Additionally, Dr. Jack Zoumaras will request that you point out specifically what changes you would like to see. This precise identification of areas to be addressed is critical in identifying anatomical causes of any issues that can be reversed with surgery.

After reviewing your medical history form, Dr. Jack Zoumaras may ask you to provide more information. It is especially important to disclose any current or past smoking habits or use of nicotine products, such as e-cigarettes, gum, or patches, as nicotine can significantly impede the healing process. He will also pay close attention to any parts of your medical history that may impact the necklifting surgery, such as uncontrolled high blood pressure, bleeding disorders, or medications and supplements that may thin the blood. It is important to have a comprehensive discussion with Dr. Jack Zoumaras regarding these matters.

After your initial consultation with Dr. Jack Zoumaras, he will assess your neck to determine if you are a suitable candidate for the procedure and to identify the specific areas that need to be addressed for optimal results. Depending on the extent of aging changes and the presence of skin wrinkles, some patients may require a laser treatment to improve the texture of their skin. Additionally, some patients may wish to explore other facial features, such as the eyelids, brows, chin, or earlobe areas.

Deep Neck Lift Procedure

A concealed incision under the chin is used perform a comprehensive neck procedure, which is the standard approach. This allows access to operate on the fat, muscle, and gland layers beneath the platysma muscle. The cervico mental and cervico mandibular angles, can then be deepend. In addition, any laxity in the platysma muscle can be addressed with plastysmaplasty. The incision is then carefully closed after the deep neck work has been completed and the skin has been repositioned. If necessary, any excess fullness from inside the mouth can be removed during the procedure. When combined with a Meloplasty or Facelift the incision is Facelift incision is used to remove excess rhytids (skin).

Deep Neck Lift Recovery

After your procedure, Dr. Jack Zoumaras will inform your companion about the surgery and they will be able to see you. It is normal to experience bruising and swelling, and you will have two drains in place, one on each side of your neck. The drains are used to minimize bruising, although some bruising may still occur. You will not require any other dressings, wraps, or straps. We ask that you spend approximately one hour in hospital recovery area before being discharged to the ward. Dr. Jack Zoumaras will follow up with you in the evening to monitor your progress, and you will be provided with his personal mobile phone number in case of any questions or concerns that may arise after hours or on weekends. may require another week for some of the bruising to lessen, those patients should plan closer to 2-3 weeks before returning to normal social activity.

Deep Neck Lift Results

In the upcoming weeks and months after your Deep Necklift procedure, you will notice a gradual reduction in the visibility of the incision line, which is concealed behind your chin and behind your ear and along the posterior hairline. During the initial stages of healing, the incision line may appear pink, but this can be easily covered with regular makeup. Within 3-4 months, the pinkness typically fades away, and the incision line begins to blend with the surrounding skin.

The longevity of the results achieved from a Deep Necklift varies depending on factors such as genetics, lifestyle, and ongoing skin care and sun protection. However, in general, this type of procedure is quite durable. While patients may seek treatment for other areas of the face such as the cheeks, eyes, or jowls as they continue to age, the area beneath the chin is unlikely to require deep approaches again. Other procedures would be more suitable for treating those areas.
